Payment for Services
- Payment Due at Time of Service: We require full payment for all services at the time of treatment. Our office accepts various payment methods, including:
- Cash
- Check
- Credit Cards (Visa, MasterCard, American Express, Discover)
- Third-party financing options (CareCredit and Cherry)
- Insurance: If you have dental insurance, we will assist in submitting claims on your behalf. However, estimated co-payments, deductibles, and non-covered services must be paid at the time of service. You are responsible for the remaining balance if your insurance does not cover the full amount.
- Payment Arrangements: For major procedures, we offer financing options. Any payment arrangements must be made before treatment begins.
Insurance Policy
- Insurance Verification: Patients must provide accurate insurance information before treatment. While we verify benefits as a courtesy, you are ultimately responsible for understanding your plan’s coverage.
- Co-pays and Deductibles: Your estimated portion is due at the time of service.
- Claims Submission: We will submit insurance claims, but the patient is responsible for any remaining balance after insurance processing.
- Non-Covered Services: You will be informed of any non-covered services and must pay in full at the time of treatment.
Payment Plans and Financing
- We offer flexible financing through CareCredit and Cherry for larger treatment plans.
- We will provide a written agreement detailing the schedule, terms, and applicable interest rates if you choose a payment plan.
Missed Appointments
- Cancellation Policy: If you need to postpone or cancel an appointment, please let us know at least three business days in advance to avoid paying a cancellation fee.
- No-Show/Late Cancellation Fee: $75 – $125 will be charged for missed appointments or cancellations for anything less than 3 business days notice. Repeated missed appointments may require prepayment for future visits.
Returned Checks
- A $35 fee will be applied to any returned checks in addition to bank fees incurred.
Patient Financial Responsibility
- Outstanding Balances: Patients will receive a statement for any outstanding balance. Payment is due within 14 days of notice.
- Collections: If a balance remains unpaid for 60 days, it may be sent to a collections agency, and the patient may be responsible for all related fees.
Refund Policy
- Billing Errors/Overpayments: A credit will be applied to future services. If no treatments remain, a refund will be issued using the original payment method.
- Treatment Adjustments: If changes occur in the treatment plan or insurance reimbursement, necessary adjustments will be communicated.
Treatment Plan Estimates
- Cost Estimates: A written estimate will be provided based on your insurance benefits and expected out-of-pocket costs for major procedures. Final charges may vary.
- Adjustments: If the actual treatment cost exceeds the estimate, we will notify you, and the additional amount will be due immediately.
